They are determined by a secret algorithm with over hundreds of factors, and the algorithm changes approximately every 12 hours (I’m talking about Google here, who has the lion share of the market).
Because of the volatility of the algorithmic changes, it is impossible for an SEO firm to guarantee higher rankings. There are a wide variety of techniques used to influence rankings, and some techniques explicitly violate Google’s Terms. Even so, these techniques can be surprisingly effective (in the short term) at increasing rankings. These techniques are called “black hat” and are risky, because although they may work today, you may be banned forever from the results tomarrow.
